Controlled trials and reviews do suggest reflexology may ease anxiety, improve sleep quality and lift quality of life in certain groups, such as people having cancer treatment or hospital procedures. However, many of those studies are small, short and at risk of bias, and the better-designed trials tend to show weaker results. So the honest answer is this: expect to feel calmer and lighter on your feet, not to fix a medical problem. If you have a health condition, reflexology can sit alongside your doctor’s care, never in place of it.

What does reflexology claim to do?

Reflexology applies pressure, mostly with the thumbs and fingers, to specific areas of the feet and sometimes the hands. Practitioners follow foot “maps” in which each area is said to relate to a part of the body, such as the toes to the head. The CAM-Cancer evidence summary notes that these maps were developed by Eunice Ingham in the 20th century, building on William Fitzgerald’s earlier “zone therapy”.

The claims made for reflexology vary widely. According to the same review, practitioners have said it can reduce stress, eliminate toxins, rebalance energy, improve circulation and help the body regulate itself. Suggested mechanisms range from blocked energy channels to effects on the nervous and hormonal systems, but the review describes all of these as theories that have not been established.

That gap between claim and proof is the real answer to “does reflexology work”. It depends entirely on what you expect it to do.

What do controlled studies and reviews actually show?

No convincing proof that it treats disease

The most widely quoted review is by Professor Edzard Ernst, published in the Medical Journal of Australia in 2009. He examined 18 randomised controlled trials covering conditions including asthma, back pain, diabetes, cancer, irritable bowel syndrome and multiple sclerosis. Only five trials reported positive results, many were small or poorly designed, and most of the higher-quality trials found no benefit.

His conclusion was that the best available evidence does not convincingly show reflexology to be an effective treatment for any medical condition. NCCIH reaches a similar view: a small amount of research suggests reflexology might ease the burning or prickling sensations linked to multiple sclerosis, but there is not enough evidence for most MS symptoms, and too little to say whether it helps irritable bowel syndrome.

More promising: anxiety, mood and sleep

Research on how people feel, rather than on curing illness, is more encouraging. A 2020 meta-analysis in the journal Evidence-Based Complementary and Alternative Medicine pooled 26 randomised trials with 2,366 adults. It found foot reflexology was linked to lower depression and anxiety scores and better sleep quality, in groups such as cardiac patients, people on haemodialysis, cancer patients and women after childbirth.

The authors were candid about the limits. Results varied a great deal between studies, only one trial blinded participants, outcomes were mostly self-reported, and follow-up was short. The signal is worth taking seriously, but it is not strong enough to call proven.

In cancer care

The CAM-Cancer review, fully revised in October 2023, found that reflexology may improve quality of life, with some positive findings for fatigue and pain in people with cancer. It rated the evidence as limited by small samples, risk of bias and inconsistent results. A US trial in women with advanced breast cancer, described by NCCIH, found reflexology eased shortness of breath but did not improve nausea or pain.

Claim vs evidence: a quick reference table

Claim

What the evidence says

Helps you relax and feel calmer

Plausible and consistent with trials on anxiety, though most trials are small and unblinded.

Reduces anxiety

A meta-analysis of 16 trials found lower anxiety scores, but results varied widely between studies.

Improves sleep

Pooled trials suggest better sleep quality, with longer sessions linked to bigger improvements. Evidence quality is modest.

Eases pain

Mixed. Some positive findings in cancer care, while a large breast cancer trial found no effect on pain.

Improves quality of life

May help some groups, such as people with cancer. Certainty of evidence is low.

Treats conditions such as diabetes, asthma or IBS

Not supported. Higher-quality trials have generally found no benefit.

Detoxes the body or rebalances energy

No evidence. These are proposed theories, not demonstrated effects.

Detects health problems through tender spots on the feet

Not supported. Reviewers warn this can produce false positive and false negative results.

If the evidence is mixed, why does reflexology feel so good?

Even sceptical researchers agree on this part. In his review, Ernst wrote that there is little doubt a foot massage is pleasant and relaxing. Firm, rhythmic pressure on tired feet, a quiet room and an unhurried hour away from your phone all help your body settle.

There are also what researchers call non-specific effects: the attention of a therapist, the expectation of feeling better and simply taking time to rest. Ernst suggested reflexology’s popularity may partly come from a strong relationship with practitioners who listen and give people time. These effects are not “fake”. They are a genuine reason a good session leaves you feeling better, even if they are not unique to reflexology.

If you spend long hours standing at work or sitting at a desk, that comfort alone can be reason enough. Our guide to tired, aching legs from standing or sitting all day looks at this in more detail.

Who should be careful with reflexology?

Reflexology generally has a good safety record. The CAM-Cancer review reports that side effects are rare and mild, such as foot discomfort, nausea or disturbed sleep, although it adds that safety data is limited. Check with your doctor first, or skip a session, if any of these apply to you:

Gout, leg ulcers, open wounds, bruising or an infection on your feet.

Poor circulation in your legs, deep vein thrombosis or a history of blood clots.

Lymphoedema.

Diabetes treated with insulin or any other long-term condition. Reviewers note a theoretical interaction with insulin, though no cases have been reported.

Pregnancy, a current injury, or surgery in the past three months. FootNote does not take bookings in these cases.

The bigger risk is using reflexology instead of medical care. Ernst warned that relying on it for serious illness, or trusting it to diagnose problems, can be dangerous. A tender spot on your arch is not a sign that something is wrong with an organ. If you have new or worrying symptoms, see a doctor.

How can you get the most out of a reflexology session?

Think of reflexology as recovery time rather than treatment. A few simple habits help:

Be clear about your goal: comfort and relaxation, not a cure.

Tell your therapist about any health conditions, medication or foot problems before you start.

Speak up about pressure. Some spots can feel tender, but it should never feel like sharp pain.

Allow enough time. In the 2020 meta-analysis, longer sessions were linked to greater improvements in sleep quality, so a quick 30 minutes may not be enough if better rest is your aim.

Rest and rehydrate afterwards. Our tips on what to take note of after a massage apply here too.

At FootNote, our Signature Foot Reflexology is offered as a relaxing treatment, not a medical one. Every session begins with a complimentary organic foot soak, and prices are nett: $38 for 30 minutes, $48 for 45 minutes, $58 for 60 minutes and $76 for 90 minutes, with member rates from $33. Detoxing, eliminating toxins and rebalancing energy are among the claims made by some practitioners, but the CAM-Cancer evidence review describes these as unproven theories.
