This applies <strong>even if one parent has sole custody<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">You may need to submit to ICA:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A <strong>completed Declaration Form<\/strong> signed by both parents (<a class=\"\" href=\"https:\/\/www.ica.gov.sg\/docs\/default-source\/ica\/forms\/declaration---change-of-name-for-minors-updated(under-21-years-old).pdf\">ICA Form \u2013 PDF<\/a>);<\/li>\n\n\n\n<li>A <strong>court order<\/strong> authorising the name change, if one parent is not consenting or uncontactable;<\/li>\n<\/ul>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><strong>Note:<\/strong> ICA reviews each case individually and may request additional documents before accepting the name change.<\/p>\n<\/blockquote>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-for-children-aged-21-and-above\"><strong>For Children Aged 21 and Above:<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">If your child is <strong>21 years old or above<\/strong>, they must:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Execute the Deed Poll <strong>in person<\/strong>;<\/li>\n\n\n\n<li>Present their <strong>original NRIC or Passport<\/strong> for identity verification.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-can-my-child-attend-if-i-prefer\"><strong>Can My Child Attend If I Prefer?<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Yes, if you prefer your child to be present, or if your child wishes to be involved in the process (especially older minors), you may bring them along.
