INTRODUCTION

Acne Scar & Scar Removal in Singapore

Acne is highly prevalent in Singapore, affecting up to 88% of adolescents. As a result, acne scars are also a common skin concern. Active acne destroys the collagen in our skin, leaving behind these depressions in the skin even after the acne heals and clears up. This is why we always advocate early and appropriate treatment of active acne in Singapore.

Acne scars come in different forms and different acne scars require different acne scar treatment modalities. It is important to note that acne scars improve by collagen stimulation of the deep (reticular) dermis and this can take time for results to show, but appropriate treatment can yield visible results from the very first acne scar treatment! Acne scar treatments can yield complete removal of acne scars, contrary to popular belief that acne scars can only be improved and not be removed completely. Run-of-the-mill and single dimension acne scar treatments in Singapore frequently give very limited improvement, which many people will try to convince is the best you can achieve.

Rolling Scars

Rolling scars have the appearance of undulating hills. These scars form as a result of fibrous bands below the skin’s surface that tugs on the top layer of the skin to pull it down. Separation of scar tissue and intense collagen are required for treating rolling scars.

Box Scars

Boxcar scars look like sharp cliffs with defined edges. Compared to ice pick scars, boxcar scars are more shallow but wider. Aside from acne, boxcar scars can also form as a result of chickenpox.

Ice-Pick Scars

Ice pick scars look like deep valleys. These depressed (atrophic) scars are very narrow and deep, sometimes extending by a few millimeters into the second layer of the skin.

Hypertrophic or Keloid Scars

A less common type of acne scar in Singapore you see with acne scars are known as hypertrophic scars. Instead of dented skin areas, hypertrophic scars are thick raised scars that protrude out from the skin. Keloid scars more commonly arise from surgeries (especially common after breast augmentation surgery) or body piercings, although it can also be caused by severe acne.

At Veritas, keloid scars are treated with less invasive treatment modalities such as lasers, radiofrequency or injectables. Surgical excision of keloid is not recommended due to its high recurrence rate of 45% or more.

EVIDENCE-BASED METHODS

Acne Scar Removal Treatment

As most acne scar sufferers have a mixture of scars and rarely just one type of scar, acne scar removal works best with a combination treatment approach tailored to each acne scar type. Some acne scars only require laser treatments or energy device resurfacing, while other scars may require paired treatments with subcision or injectables. Run of the mill and mass market acne scar treatments will give average results at best, and we frequently see patients who have been through multiple treatment sessions from such mass market acne scar treatments that yield no results at all! Remember that many treatment centres in Singapore will tell you that scars can only be reduced and not removed. This is normally due to limits in treatment equipment, knowledge and technical skills.

The synergistic combinations of acne scar treatments and the timings between the various acne scar removal combinations are a carefully developed clinical science which is precisely tailored for each individual with respect to both their scar type and skin type. Subcision

Subcision is a treatment that breaks apart fibrous bands/tethers formed during the inflammatory acne process and left behind after the healing phase. Subcision releases tethered acne scars and is highly recommended to follow up with procedures like fractional CO2 laser. Fractional CO2 laser stimulates collagen.

Skinboosters and Collagen Stimulators

Skinboosters such as PDRN Healer, Polynucleotide Skin Booster, as well as collagen stimulators combined with Subcision and injected into the scar tissue – a must for certain (but not all!) acne scar types in Singapore. This procedure requires skilled techniques but give drastic improvements when done properly.

Chemical Peels

Medical-grade chemical peels such as TCA cross, AHA, and salicylic acid may be used in combination with lasers to improve pitted scars.

Why You Should Treat Your Acne Scars Early

Conventional thinking might suggest you “wait” to see if your acne scars improve over time. In reality, waiting means your scars may worsen over the years and take longer to treat. Depending on the location of your scar, acne scars can worsen in appearance as you age, due to reduced collagen and elastin in the skin. Depressed or atrophic scars are scars that tend to look worse as you enter your 30s and 40s. Certain areas of the face such as the temples and mid cheek experience fat loss as we age. As volume loss happens in these areas, the appearance of depressed scars is accentuated.

Additionally, older skin requires much more stimulation to produce enough collagen to smooth out scars. Acne scars left alone naturally become costlier to treat since a greater number of sessions are required to stimulate collagen for scar improvement.
