Thinking about getting anti-ageing treatments in Singapore? If you have been researching non-invasive skin tightening procedures, you have likely come across popular options such as Thermage and HIFU. Both aim to lift and tighten the skin without surgery but in fact, they work differently and deliver different results.
How Thermage Works
Thermage uses monopolar radiofrequency energy (RF) to heat and target skin at the dermis to the subcutaneous fat layer between 40°C and 75°C. This causes an intentional thermal injury that triggers a biphasic response. The heat causes collagen fibres to undergo denaturation. When collagen fibres contract, they strengthen and tighten, resulting in a skin tightening effect. The second response – neocollagen synthesis and remodelling, occurs over the next few months as part of the body’s wound healing mechanism.
How HIFU Works
HIFU (High-Intensity Focused Ultrasound) works by delivering focused ultrasound waves to deeper layers, including the SMAS layer — the layer that is tightened in a traditional face lift surgery. Temperatures between 65 to 75°C are delivered into the targeted zones. This process similarly causes contractions and tighter shelving of new collagen. Through neo-collagenesis, wrinkle elimination and skin tightening are achieved.
Thermage and HIFU Difference
A key difference between Thermage and HIFU lies in the layers targeted. Thermage targets the more superficial skin layers (dermis and subcutaneous fat layers), making it suitable for superficial skin concerns such as fine lines and mild sagging. HIFU targets multiple skin layers, from the uppermost layer of the dermis till the SMAS layer, providing a stronger lifting effect. This makes HIFU suitable for those with more pronounced sagging and wrinkles.
From a clinical perspective, I tend to favour HIFU for face lifting and tightening for several reasons. One key consideration is treatment comfort. While Thermage is often described as moderately to significantly uncomfortable, HIFU MPT is generally very well tolerated by most patients, with the sensation likened to a muscle ache or rubber band snapping. Higher comfort levels allow the practitioner to perform sufficient passes when needed, and deliver more complete treatments. Check out the article on non-surgical facelift treatments by my colleague.
When choosing a skin tightening or lifting treatment, many patients worry about unwanted facial fat loss, which can lead to a hollow or overly gaunt appearance. This brings me to my second reason. Because Thermage uses RF energy to heat tissues in a diffuse, non-layer specific way, its coverage is broader, but there is lower precision. HIFU, in contrast, delivers targeted energy with various depth selection, allowing doctors to target specific layers, offering more control, and thus lower risk of unintentional facial fat loss.
As mentioned, because HIFU energy is controlled and targeted at fixed depths, this enables the doctor to select exact treatment layers and avoid/skip the fat pad layers and areas where fat loss could cause hollowing. With Thermage, diffuse volumetric heating used is more uncontrolled; energy spreads through skin and fat less predictably and does not allow true layer by layer targeting.
From a safety standpoint, targeted energy delivery = less collateral fat heating and risk of skin burns. During Thermage treatment, diffuse heating with RF means that the tissue temperature rises over a larger volume, not at specific focal points, which increase risks in patients with thinner skin or low fat reserves. HIFU on the other hand creates targeted thermal coagulation points, allowing us to bypass certain zones, such as fat pads. Facial fat pads migrate and redistribute as we age, which can make fats superficial and uneven. Since HIFU pulses can be focused at specified focal points and depths, this enables us to bypass the fat layers and focus the energy at the dermis and SMAS layer during skin lifting and tightening treatment, something that is not achievable with Thermage, given selective heating is not possible due to its diffuse heating pattern.
Note that this does not mean:
– Thermage always causes unwanted facial fat loss
– HIFU can never cause facial fat loss
The skill and judgement of the doctor matter as much as the device used.
Neither treatment is “good” or “bad” by default. Treatment outcomes depend on how energy is delivered, which tissue layers are targeted, and how treatments are planned for each individual’s unique facial structure. Both devices are safe when used by experienced doctors. The difference is that HIFU allows fat avoidance by design while Thermage requires fat avoidance by restraint.
