If you have lost your Deed Poll created by Loh Eben Ong LLP and it remains in our archive, you can apply online for a certified true copy (CTC). This service is only for Deed Polls done through Loh Eben Ong LLP. The application is subject to the availability of your Deed Poll in our archive and our terms. Do not use this form to apply for a Deed Poll for a name change. The process includes submitting the form online, booking an appointment, and completing payment upon attendance, preferably contactless.
How to Apply for a Lost Deed Poll CTC Online
Have you lost a Deed Poll processed through Loh Eben Ong LLP? If it is still stored in our archive, you can apply online for an additional certified true copy (CTC).
Eligibility:
- Only Deed Polls done through Loh Eben Ong LLP are eligible.
- Do not apply if your Deed Poll was done through another law firm.
- We do not guarantee that your Deed Poll is still in our archive. We also reserve the right to reject an application without giving a reason.
- You must bring proper identification documents, or a written authorisation, when you collect the copy.
- In addition, we charge a search fee and a certification fee.

- This form is not for applications for a Deed Poll for a change of name.

What happens if my deed poll is not in the archive?
We do not keep originals, and we cannot guarantee that a copy survives. Nevertheless, you may execute a fresh deed poll that recites the earlier change of name, and our Deed Poll Online service prepares it.
Can I apply for a lost deed poll CTC for a relative?
Yes, provided the deed poll holder authorises you in writing. In addition, we must verify both identities at collection.
Do you post the certified true copy?
No. You collect it at our office, because we must check identity at the point of collection.
How many sets should I order?
Order one set for each institution that will keep a copy. Therefore, ordering extra sets now costs less than returning later.
Is this the right form for a name change?
